Care Coordination

Care coordinators develop a personalized care plan for each breast cancer patient to help decrease barriers to treatment and provide linkages to support services. This program prioritizes metastatic breast cancer patients, low-income, uninsured and underserved patients, as well as breast cancer patients facing physical or transportation barriers.
